For this, you'll want to use 5.6.0 of the extension; it would appear, it may not have shown up in the Den, so I just enabled it now in case it didn't...

Thanks for the additional information. I've logged this as an item in TFS (https://github.com/Inedo/bmx-tfs2012/issues/16), and we'll fix it ASAP. In the mean time, it might be easy to work-around with doing a clone/patch: http://inedo.com/support/tutorials/cloning-and-patching-an-inedo-extension.

There shouldn't be. It's probably trying to create a new record for whatever reason and that is triggering the dupe check. There's no other provider with that name.

We've added support for extensions to allow HTML issue descriptions as of 4.1.0, but we still need to update the TFS extension to indicate this. We should have it available within a day or two.
