How can we find out who actually created the deployment plan?

Is there an audit trail of all the changes applied to a deployment plan and who made them?

The general audit/event log (found on the admin page) contains events of when the deployment plan is changed and created, but the plans are not versioned.
This is part of our 5.0 plans, however.

Is there a limit on how far back the event log will capture?

It goes back to Day 1 of your install; you can query the table directly (it's [EventOccurences]) if you need to do some complex searching.
